What Insurance is Accepted?
CIGNA, AETNA, Blue Cross Blue Shield Anthem, Medicaid, and United Healthcare. We will also see those who are uninsured.
What Do I bring?
A printed copy of your child's immunization record.
Your child's insurance card.
Proof of residency or ID.
IMMUNIZATION AND HEALTH CERTIFICATES
Georgia law requires a student, regardless of their grade level, who has never been enrolled in a Georgia public school, to provide an Eye, Ear, Dental, Nutrition Exam Certification (Form 3300).
Georgia law also requires children attending school (Kindergarten -12th grade or child care facility) to be age-appropriately immunized with all the required vaccines at the time of first entry in school. A new entrant is a child entering a school in Georgia for the first time or entering after having been absent from a Georgia school for more than 12 months or one school year. All students, regardless of grade and including foreign exchange students, must have the Georgia Department of Public Health Certificate of Immunization. NOTE: Only health departments and physicians licensed in Georgia can obtain blank immunization certificates (Form 3231). Take your child's personal immunization record to a health department or Georgia physician and they can complete the form and give any required vaccines. (Form 3231)
REQUIRED IMMUNIZATIONS
All children entering a school or day care (childcare) facility operating in the state of Georgia must be immunized against the following diseases:
